The scanner has been fixed, thanks mom! ;)  Anyways, this pdf is pretty self-explanatory.  The cut off text says "Special considerations: contacts at #s 2-4, wire hole at #s 2-4, corner cuts at #s 1-3".

I forgot to mention this; use the solder to make your contacts wider.  Just put a drop or two in each of the slots.

... and because this image was too big to be attached to the previous post, here is what the finished product looked like for me.  It isn't the best glue/solder job, but it still works.
