I have tried a 1GB, 4GB and 8GB flash cards, windows and mac to copy the chdk files to the card, copied everything extracted from the zip file to the root of the flash card (image attached).  I cannot get this to load with the card locked and I get no Firmware menu in play mode.  Any help would be greatly appreciated.

you don't say how you set up the card, but can I suggest you use my STICK app (on a Mac is simpler, but it works under Windows too). That will make sure that the card is 'bootable' and has the right format for CHDK to automatically start when the camera is switched on. See

You appear to have missed the step where you need to make the card bootable.

The advice to use STICK given above it good advice.
