Is there any way to reformat an SD card after CHDK has been installed and return it to original formatting (FAT or FAT32)?  Or is the SD card lost forever to any other use than for CHDK?

I tried to do this using the format right-click command in WinXP Explorer but got an error message saying "no card found".

If card has only 1 partition: format in camera.
Otherwise, use same tool you used to multi-partition it to delete patitions, then format with camera or XP

example with aefdisk:

first determine device number of your card:
"aefdisk32 [card-device-no] /show"  using 1, 2, etc as card-device-no
once you're sure you got the right device number (look at the size!), do
"aefdisk32 [card-device-no] /delall"
and your card will again be totally blank.

hope that helps,

Just do a low level format inside the camera, that will wipe everything out, even multiple partitions.
