I have an Ixus 100IS (or SD780) which has one of the dual function USB/Video outputs. There are various sites out there that point out if you get your hands on a connector like this one http://www.sparkfun.com/products/9167 (also known as ExtUSB and used by HTC) you can use the USB pins to trigger capture, and the video pins can still be used as output.

However, I want to be able to use PTP to trigger the camera using the computer and download the image straight away. Therefore I would need all four USB pins and a USB connection operational (as opposed to just the two signal pins required for USB remote cables). Has anyone tested this as working, so that you can have the live video and the USB connection running at the same time?

While the device also has HDMI, it's only active during display mode (so anyone has ideas on getting that working during record mode, they're also welcome)

We had the same problem with our SX220HS, a combined USB and video out plug, and yes they do work at the same time!
As webguy mentioned already, canon did not invent this plug by themselves and you don't have to solder your own plug.

Instead we bought this HTC YC A300 adaptor  on amazon:

If you use the HTC YC A300, you need to grind the USB plug a little to make it fit, but this is no problem.
Then, you will need a 2.5mm jack to 3 cinch cable, and short the yellow cinch. The white cinch is your video signal and then you have it, live preview and complete usb functions using ptp at the same time!

I have an s95 and bought a similar connector - see here
s95 multiway connector

It provides a normal USB socket as well as a socket for a 3.5 jack plug. To activate video you have to solder a wire between pins 6 and 11 and to make the plug fit into the camera you have to do a bit of filing, but both are very easy.
